We welcome you to be the newest member of Chino Veterinary Hospital family. Here are a few things to expect for your first visit:
- If we’ve never seen your pet before, please fill out our online ’new patient questionnaire.’ For your convenience, you can fill this out online, or at our office.
- Call our office to schedule an appointment (909) 628-5155. Our receptionist will work with you to find the best appointment time that works with your schedule. Remember, we’re open Saturday’s too.
- For the safety of your pet, please make sure that all cats are in a carrier, and all dogs are kept on a leash.
